Liability Waiver

By signing up for and participating in soccer activities organized by Extra Time Athletics, I acknowledge that soccer is a physical activity that carries an inherent risk of injury.

I voluntarily assume all risks associated with participation and release Extra Time Athletics, its organizers, and the host venue from any liability for injuries or damages arising from my participation, to the fullest extent permitted by law.

I confirm that I am 18 years of age or older, physically able to participate, and responsible for my own health insurance coverage.

I am the parent or legal guardian of the player being registered, and I am registering them with my permission and on their behalf.

I acknowledge that soccer is a physical activity carrying an inherent risk of injury, and I accept those risks on the player's behalf. I release Extra Time Athletics, its organizers, and the host venue from any liability for injuries or damages arising from the player's participation, to the fullest extent permitted by law.

I confirm the player is physically able to participate, that I am responsible for their health insurance coverage, and that I or another responsible adult will be reachable at the phone number given at registration for the duration of the event.

I agree that photographs and video taken at Extra Time Athletics events may be used on our website and social channels. For a player under 18, the parent or guardian agrees on their behalf. Tell us at check-in if you'd rather not appear and we'll respect it.
